Breast augmentation

At Reformkliniken, we are proud to offer our patients the latest technology and methods in breast augmentation. With over 5000 successfully completed breast surgeries, we have the experience and expertise necessary to realize your wishes and goals.

For most people, a good result means that you don't have to think about the breasts and that they feel like a completely natural part of the body.
Breasts are an important part of self-image but they are also important for how you want to appear to others. To feel confident and comfortable in different situations, both with and without clothes, it facilitates that the breasts have a good shape and stability.

Depending on the conditions and wishes, the breasts can become much or slightly larger and sometimes not at all larger because it is possible to combine a reduction of the breast with the insertion of implants.

For most people, a good result means that you don't have to think about the breasts and that they feel like a completely natural part of the body.

The reason why women do not thrive can be innate characteristics such as the breasts being small, uneven or perceived as aberrant, but it can also be the result of changes associated with pregnancy, breastfeeding, or weight changes for other reasons. The risk of complications is very low and the material in breast implants is more durable today than before.

This allows most people to get a good result with one procedure. The body changes over time and is affected by pregnancy and weight change even after surgery so it is important to be aware that you may need further procedures in the future.

If the breasts have a good shape and the skin is of good quality, it is enough to insert an implant but if the skin is stretched and the breasts are sagging, a breast lift may be required at the same time.

In the case of a large difference in the size and shape of the breast, so-called “asymmetry”, it may sometimes be sufficient to use implants of different sizes but it may also be necessary to have a breast lift at the same time.

Breast Implants — Round and drop-shaped implants

A round implant is like a circle in the bottom plate, the part placed against the chest wall, while the shape adapts according to how much space it has. A drop-shaped implant has a width, height and projection and they are made to hold a definite shape in the body.

For those who want a lifelike result, a drop-shaped breast may be ideal, but a round implant can have a droplet shape in the body.

For example, a round implant can be placed in the body through a small opening in the armpit while a drop-shaped implant requires a larger scar to be placed in the crease under the breast.

Implants

Silicone Implants

Modern breast implants are made of silicone of a type known in Sweden as “jelly raspberry”, in Denmark “skaerefast” and in English-speaking countries “gummy bear” or “cohesive”. This means that the contents are soft but not liquid. If the implant should be damaged, the contents stay in place instead of flowing out. This helps the breasts to keep their shape better and it reduces the risk of complications.

When a foreign material is placed in the body, the healing creates a membrane called a “pocket” that helps keep the implant in place and has the desired shape.

The pocket forms and stabilizes in the first four weeks but it can be affected up to about three months after surgery. Therefore, the first time after surgery is very important for how the breasts will look later.

Normally, the pocket is thin and permissive, which means that the implants are perceived as soft and a natural part of the body. If instead the pocket becomes tight and thick, the chest will be perceived as tight and uncomfortable. The pocket should be spacious enough to allow the implant to lie flat. If the pocket becomes too small and tight, the implant will be affected in shape and have folds on the surface. If the pocket becomes thicker and tighter, the shape is increasingly affected and the chest becomes tighter until it is like a small ball. This is called capsule formation and is a rare complication that can manifest itself early after surgery or only after many years.

The surface of an implant can be smooth or frosted. Frosted implants are also called “textured” and the surface can be anything from heavily roughened to almost completely smooth in feel.

A heavily textured surface should cause the implant to “grow firmly”, a reaction between the body and the implant, which should theoretically make the implant stay in place better. Unfortunately, this does not work as well in practice, but the implants can have a so-called double capsule that allows it to move freely inside its pocket. A membrane has formed on the outside of the implant and a membrane on the inside of the body.

Heavily textured implants also have a stronger link to the highly unusual tumor form, BIA-ALCL, which has gained recent attention.

Size of Implant

To find the right size of the implants, we measure the width of the chest, the location of the nipples and the volume of the own breasts. We also check the quality of the skin, the shape of the chest and the position of the breast folds. All these factors influence the outcome and it is important to be aware of your own prerequisites from the start.

The width of the implant should be adjusted according to the conditions of your body and your wishes. For those who want a lifelike result, the size of the implant should match the width of the chest. For those who want a more exaggerated and unnatural result, the size of the implant should be too large so that it fills outside the chest but also goes higher up the chest wall.

What we have the least opportunity to influence is the distance between the breasts in the middle, the so-called gap. The width of the gap depends on how close the breasts sit initially and how much breast tissue and subcutaneous fat you have that can hide the implant.

How does an operation work?

An appointment for a consultation with one of our plastic surgeons is booked to 30 minutes. We go over your health and if there is anything special to consider in terms of general anesthesia and surgery. The most important thing is that you should get a good picture of your prerequisites in terms of surgery and what to expect.

It is good that you prepare yourself so that you know what result you want and feel free to bring pictures to your consultation so that we can discuss these.

We examine and measure your breasts to have a good starting position and be able to advise you on the best possible course of action.

Normally, one visit before surgery is enough, but if you feel uncertain about something, you will come back as many times as you need to feel confident before the operation. We also keep in touch before and after surgery so you can ask questions directly to your plastic surgeon or get help from someone on our dedicated staff.

On the day of your surgery, you come to the clinic and have a shower before the surgery and after that you dress in the clinic's dressing gown and can rest in a bed. We do everything possible to reduce the risk of complications and that includes careful hygiene.

You will receive sedative and relaxing medication if you wish, but also pain relievers to ensure that you feel as good as possible during and after surgery.

We wish you to stay overnight after a breast augmentation to ensure the best possible results and peace of mind. This is included in the cost of surgery.

You will see your plastic surgeon or nurse at follow-up after 1-2 weeks depending on the procedure. After this, we stay in touch to be able to advise on how to take care of yourself in the best way.

You book an appointment that suits you about 6-12 months after the operation so that we can evaluate the result and see an early final result. It takes about a year before it can be considered more complete in terms of scars and that the body has fully adapted to the change.

Combining interventions

What are the risks of facelift surgery

Bleeding, infection, and wound healing problems are the most common surgical complications. The risk is minimal but for those affected it is a great strain. We do everything possible to avoid and prevent complications and it is important that the person who is operated on does so as well.

Smoking doubles the risk of all kinds of complications. In order for the body to heal as well as possible, it takes four weeks completely without smoking. Alcohol also increases the risk of healing problems and it is important to be very restrained about two weeks before surgery.

What are not actual complications but are a bad result are, for example: “DB”, “Bottoming out”, “rippling” and muscle strain

Fluid retention

The space between the abdominal wall and the skin is a wound surface that leaks fluid until the body “glues” it together. This occurs in a balance where the body also absorbs the fluid. By taking it easy initially and wearing a bandage, you can help the body minimize the propensity to collect free fluid. If a significant amount of free fluid accumulates between the abdominal wall and the skin, it can be drained during a clinic visit. Removing the fluid is beneficial for uncomplicated healing, but it is not a sign of a serious problem.

Decreased sensation

When the skin and fat are detached from the abdominal wall, nerve branches are cut off, leaving the skin feeling numb. The sensation gradually recurs in the first few months, but it can remain permanently reduced, especially in the lower middle of the abdomen near the scar.

Scarring

An abdominoplasty requires long incisions to remove excess skin and avoid irregularities at the ends of the scars, known as “dog ears,” which occur when there is too much tissue left. The appearance of scars depends largely on the body's ability to heal and our goal is to achieve the best possible results with thin, pale scars. Some people tend to form a lot of scar tissue, and it takes a long time for the scars to mature. If undesirable results occur, they can be corrected as much as possible, often easily done under local anesthesia at no cost.

Blood clotting

After major interventions, the body's balance between blood flow and clotting is disturbed, increasing the risk of blood pooling in the veins of the legs and forming clots that can travel to the lungs. For each surgery, we assess the risk of a blood clot and the benefits and risks of medications that prevent blood clots. Because there are significant individual differences between patients and risks with treatment, we discuss this with you and select the best possible treatment.

Hemorrhages

The wound area is large on the inside, and many blood vessels are cut off during surgery. There is a risk that a blood vessel may open up after surgery and cause significant bleeding. This risk is highest after significant weight loss where large blood vessels previously supplied the skin with blood. The risk is greatest in the first few hours after surgery so it is important to have good follow-up during this time.

Rippling

Rippling as an undesirable result means that there will be a noticeable waviness on the implant. When the implant is on a smooth surface, it has a smooth shape but when placed in the body it is always wavy at the edges. Implants can be different in shape, round or drop shaped and they can be different solid but all implants have rippling in the body.

If the implant is covered with skin and muscle, this is not noticeable, but if it is thin between the implant and the skin surface, you can notice the irregularities more easily. It is important to consider this in advance when choosing implants and the main factor that increases the risk of noticeable rippling is the size relative to the dimensions of the body and the quality of the skin.

Muscle impact

This can mean many different things, but when it comes to undesirable results, it's about both form and discomfort. All breasts and implants move with activity in the pectoral muscle.

A typical effect of implants behind the breast muscle is that they are pulled upwards and outwards as you tense the pectoral muscle and then the breast deforms but it regains its shape when you relax. An implant in front of the muscle is in contact with the front of the pectoral muscle and also moves when you tense the muscle but it does not become deformed in the same way. In rare cases, it is associated with pain when using the muscle. If the discomfort is pronounced, it may be appropriate to move the implants and place them in front of the muscle.

In the case of complications and undesirable results, it is included in the warranty of the clinic and corrected free of charge. It is important to know that not everyone can get the same result and the goal is the “best possible result”. At the consultation we point out what is special for you and if there is something in your circumstances that we must especially consider.

Bottoming out

This means that the chest stretches on the underside causing it to sink down and the chest to become increasingly thin on the underside. It can be difficult to solve because it is about the strength of your own body, but it may be appropriate to change the implant, sew the pocket where the implant is located or do a breast lift.

Double Bubble

DB or Double Bubble means that the breast is not perceived as a unit but the chest hangs on the outside and has a visible remnant of the old breast fold. The risk of this is greatest if the breast has poor skin quality and the fold is tight and the implant is located below the breast fold. Sometimes it can be solved by placing the implant in front of the muscle instead of behind but many times it takes a breast lift to break the old fold
